This person should be comfortable managing details, asking the right questions, and helping families feel supported through every step of the care journey.

What this role would support

The Care Coordinator would help guide families and care partners through the intake, communication, and coordination process. This role may support referral follow-up, family communication, care documentation, scheduling coordination, nurse communication, and ongoing service updates.

This person would play an important role in helping Larkin Life Care create a care experience that feels personal, organized, and dependable.
